You can either book an appointment with the appropriate healthcare professional or alternatively please phone the reception desk on 01506 414586 to arrange your appointment.
Please note you will usually be contacted by the Practice , to invite you to attend for your diabetic review with the Nursing Team.
Dr Smith and Dr Taylor both have an interest in diabetes and you may also be referred to them as part of your review.
